Moving from theory to practice involves implementing a new care program. In this case, the experience was on applying a learning supported model designed to increase patient autonomy and involvement in care. In this case, the justification for the model was that it uses patient-centered approaches that increase patient participation and improve care. For that matter, the model perceives and understands each patient as a unique individual with a story that is the core of their care (Hermida & Sánchez-Herrera, 2018). There was a perception among the nurses and other medical personnel that the previous care model did not give patients enough support to be involve and take responsibility for their health and treatment. This mutual impression of the model and importance of change, it was easier to apply the new model as it received support from nurses and other medical personnel. This intent to change was articulated by the leaders who ensured that there were adequate resources to support the change. Second, the experience highlighted the importance of supervision and reflection. This allowed for the application of the model to be evaluated and continuously improved to achieve the desired outcomes. The supervision ensures that the new model is applied as intended while reflection ensures that what may have gone wrong can be improved on over time. Third, the application of the model brought about changes within the practice environment. At the beginning, the model was perceived as abstract and difficult to understand. But as it was applied, it became more familiar thereby allowing the personnel to move from theory to practice and increase their understanding of practice in relation to the model. From the initial feeling that change is difficult, the personnel went on to embrace the change, actively worked on it and no longer required supervision (Reinholz & Andrews, 2020).
